What To Expect From Your Wellness Visit
Wellness Visits
Wellness Visits are annual exams you have with your Physicians and Advanced Practitioners to your personal medical history, to gather medical information, and assess your risk factors for developing certain medical conditions. Wellness Visits are now covered by commercial insurers under the Affordable Care Act.
Examples of what will be discussed during this visit are:

Past medical, social, and family history

Complete physical exam and review of body systems

Review of medications
-
Immunizations
-
Risk factors
-
Review of age/gender appropriate screening tests
* Laboratory and other diagnostic tests may require co-payments
Problems or Concerns
Some abnormalities are found during Wellness Visits or problems/concerns are discussed that are not covered under the scope of a Wellness Visit.
Although the problems or concerns are addressed during your Wellness Visit, such problem(s) identified are often not recognized by health plans as part of a Wellness Visit and CMG is obligated to bill separately for this service.
Know your Insurance
PRemember that your provider does not know your health plan benefits and sees many patients throughout the day with various insurance plans. It is your responsibility to know what services are covered under your health plan.
We Care About Your Health
If you or your provider feel a problem or concern needs to be addressed immediately, you may be asked to reschedule your Wellness Visit, or to schedule a Problem Visit so that your problem/concern can be addressed in the most timely manner possible.
